Sharing Your Medical Record

In line with Gloucestershire Clinical Commissioning Group policy, as a surgery, we have now made our patient records sharable. When booking your Improved Access appointment our receptionist will ask if you consent to share your record with the doctor and will record your decision.

Our doctors recommend record-sharing for the best possible care but if you don’t wish your record to be shared outside the surgery, tell the receptionist and she will note that on your medical record.

Please be aware that Improved Access GPs will need to see your record, so a sharing dissent could limit the appointments available to you.

Our registered SystmOnline users will be able to change their sharing settings within their own passworded, confidential accounts.  If you would like to register for on-line services, please attend the surgery in person with photographic proof of identity such as a passport or photographic driving licence.
